A Remediation Decision Review provides an outside expert evaluation of the remediation structure, documentation trail, and performance monitoring before final action is taken.
This review helps identify gaps in documentation, weaknesses in the remediation process, and areas that could expose the program to accreditation or legal risk.
Written feedback delivered within 72 hours of receiving complete documentation.
Followed by a private one-on-one consultation to review the findings.

REVIEW PROCESS
Step 1 — Request the review
Complete the request and receive a secure link to upload documentation.
Step 2 — Submit documentation
Provide the remediation plan and supporting materials for review.
Step 3 — Expert evaluation
The remediation structure, documentation trail, and performance monitoring are carefully evaluated.
Step 4 — Written findings
You receive a detailed written review within 72 hours of receiving complete documentation.
Step 5 — Private consultation
A one-on-one consultation is scheduled to review the findings and discuss next steps.
INFORMATION REQUESTED
To complete the review, programs will be asked to submit:
• Remediation plan documentation (PDF or Word)
• Resident PGY level
• Primary area(s) of deficiency
• Brief summary of the performance concerns
• The action the program is preparing to take (termination, non-renewal, continuation of remediation, or other action)
